The American Dental Association (ADA) has given its golden seal to brand name fluoridated toothpastes for their cavity
fighting features. It’s no surprise really – topical use of fluoride
has been proven to be effective at inhibiting tooth demineralization, promoting remineralization and inhibiting the growth of bacteria associated with tooth decay. Fluoride is marketed in the US as a bone fortifier and is administered at the dentist’s office for nearly all pre-adolescent teeth cleanings. Finally, the idea of fluoridated water may seem like a great thing to add to your new child’s water, because it fortifies teeth – but beware! What isn’t so well known is that the potential toxicity of fluoride is dangerous, and if swallowed in concentrated form or ingested over a longer period of time, it may have potentially damaging side effects.
In 1977, The Institute of Medicine (IOM) specified adequate intakes of fluoride of 0.01 mg/day for infants through 6 months, 0.05 mg/kg/day for children beyond 6 months of age, and 3 – 4 mg/day for adults, to prevent dental caries. Upper limits (UL) of 0.10 mg/kg/day in children less than 8 years old and 10 mg/day for those older than 8 are recommended for prevention of dental fluorosis. Similar levels have been endorsed by the American Dental Association (ADA, 1994) and the American Dietetic Association (ADA, 2000). What does this all mean? Fluoride has an affinity for calcified tissues. The trace amount of daily intake of fluoride is beneficial, but too much of it can result in patchy or mottled looking teeth, known as dental fluorosis (or brittle bones, known as skeletal fluorosis). Studies are needed for health researchers to investigate the relationship between intake levels and human health. 75% of our daily fluoride intake is from water or beverages, like tea and juices.
The fluoride content of a water supply is often described by the term ‘parts per million’ (ppm). 1 ppm = 0.1 milligrams of fluoride per 100 milliliters of water. About 1 ppm seems to be the optimal level of fluoride for a drinkable water supply; a litre of such water would provide 1 milligrams of fluoride. At 2 ppm, mottling of tooth enamel
may occur. The actual condition of ‘fluorosis’ or excessive deposition of fluoride in bones is only present after prolonged consumption of 20 to 80 milligrams of fluoride per day.
Repeated exposure to fluorine in naturally-occurring forms (such as in fruits and other produce) is one thing, but overexposing your child to fluoridated baby formula (identifiable by needing to add water to the formula) could put your child’s appearance – and bodily health – at risk. The key take-home message for parents is to avoid added fluoridated liquids or even foods of any kind. No level of fluoride is perfectly safe, and ideally, we wouldn’t need to use it at all – however, fluoride’s teeth strengthening properties, when used in proper doses, can be a beneficial additive.
If your child does develop dark-spotted or mottled teeth after getting his or her primary teeth, pay close attention to their diet! Nutrition should be strictly monitored for fluoride ingestion (including toothpaste!) – if so, he or she should have perfectly clear, spotless permanent teeth erupting around age 5. Experts also agree that it is a good idea to test the water from your home water lines, to see if it is overly fluoridated. Simple take-home tests are available from your local water or health bureau, or can often give past and current fluoride level readings. Do your research to protect your child’s and your health.

Although you may think acid reflux disease and your child’s dental health are unrelated, but recent studies have suggested the close relations.

Recent studies have linked acid reflux and dental erosion.
Acid reflux disease, defined as chronic symptoms or mucosal damage produced by the abnormal reflux in the esophagus, is nothing short of painful and distracting. When occurring in children, the symptoms can be even more painful and difficult to handle. Just nine days ago, the Pediatric Academic Society released findings of a study conducted by the University of California San Francisco stating children who have severe acid reflux have a much higher risk – up to six times the risk – of dental erosion and gum disease than those without acid reflux disease (or GERD). This is a unique finding and although it should be researched more before being considered fact, if your child suffers from acid reflux, be sure to keep an eye on his or her dental health!
Dental decay, erosion and sensitivity are a result of an overly acidic oral environment, and can often be quite painful and difficult for young children to handle, just like acid reflux. Regular hygiene (like brushing and flossing twice daily and after meals) is important as are regular checkups with your dental professional twice yearly, but what other things can you do to neutralize your child’s acid reflux, and in turn prevent dental and gum erosion? Besides healthy diet that consist of fresh vegetable and fruits, avoid greasy fried food , stay off sugary carbonated beverage,one physician who was a part of the aforementioned study has recommended children with acid reflux disease (even at early stages) should be started on a dental hygiene regiment, to be overlooked by a dental professional regularly, just as medical checkups are. Dental erosion can be uncomfortable and potentially hazardous to your child’s oral health, which is why prevention, early detection and adequate protection are always the best keys to health. By taking the right steps early in your child’s life (especially if your child suffers from acid reflux), you can assure your child has a better chance to live his or her life with a happy, healthy smile!

Fluoride
treatment is effective with preventing cavities and tooth decay. As a dentist, this is what I have recommended to people who I see can benefit from fluoride treatment’s ability to remineralize and desensitize your teeth. However, I must confess there is risk involved with using fluoride due to its potentially damaging systematic effects.
Fluorine, the ionized form of fluoride is highly active to phosphate ions in your body (where calcium is found in large amounts – like the bones and teeth). In teeth, fluorine combines with the inorganic compound apatite, to form a harder, less water-soluble fluoride salt, which resists the acidic erosion of some oral environments. A pregnant mother’s fluorine intake affects the primary teeth of the fetus. Fluorine intake from fluoridated water (or a fluoride supplement) can also affect teeth in child development of up to around age 8 (making teeth harder and more resistant to decay). Unfortunately, excessive amounts of fluoride can lead to fluorosis, which causes white, brown or black stains (and even pitting) of the teeth, and/or brittle, aching bones and joints. An even darker fact regarding fluoride: many researchers believe it is carcinogenic. Japanese research has shown fluoride as being capable of transforming healthy cells into cancer cells. Additionally, the New Jersey Department of Health has found that young men exposed to fluoridated water have a much higher occurrence rate of bone cancer. Other studies performed by private corporations (Procter & Gamble in particular) have shown fluoride used to fluoridate public water sources significantly increases the chances of genetic damage in its drinkers.
So use fluoride with caution – it is beneficial for cavity
prevention and treating tooth sensitivity, but it is for topical use only – do not ingest! Children especially need special care from their parents with undertaking any type of fluoride treatment, no fluoride toothpaste or fluoride should ever be swallowed. If fluoride use is recommended by your doctor or dental professional, calcium supplements (or food rich in calcium like dairy products) are recommended to ensure your bones stay strong and balance out any potential loss of calcium due to fluorine exposure.
There are alternatives for remineralizing your tooth structure, such as calcium phosphate and calcium carbonate. If you have any concerns, speak with your dental professional. Remember, the best way to prevent tooth decay is to eat sensibly and keep your mouth sparkling clean by properly brushing and flossing after every meal.

“I am scared! Am I going to loose my teeth?”
I looked at Jenny, as I have with many similar pretty young ladies, and really felt for her.
“Jenny, you will be fine – you are here to make sure you can have a healthy and beautiful smile. It is challenging, but work with me – together we can get your mouth back in shape.” And we did.
Jenny is not alone. Among the susceptible figure-conscious young female group, eating disorders are quite common, and teeth erosion is a side effect of the acidic oral environment resulting from either purging or a poor diet.
Eating disorders, including bulimia (binge eating followed by self-induced vomiting), anorexia and poor dieting habits carry side effects which are detrimental to your bodily and mental well being – they can also rot your teeth. Studies have shown that up to 2.1% of males and 7.3% of females purge (self-induced vomiting) at least once a week, and the numbers are even higher for adolescents.
Tooth surface loss from erosion caused by chemicals such as dietary, gastric or environmental acids typically wear away tooth enamel, which leads to sensitive teeth, caries or gum disease. If untreated, these effects can exacerbate leading to teeth grinding and gastric reflux disease. Soft tissue lesions, malnutrition, a suppressed immune system, low self-esteem, an addictive personality (smoking, drinking or drugs), peptic ulcers and esophagitis are common among people who suffer from bulimia and anorexia. Oral damage from such disorders can include a severe breakdown of the teeth or their supporting structures which can affect the facial muscular system, negatively affecting your daily quality of life; the effort to restore a damaged mouth can become extremely challenging and costly.
Treatment of the problem, besides education and being supportive to the person suffering from the disorder, involves precise dental management; controlling the oral environment, drinking water or healthier juice instead of diet soda, eating healthier (as opposed to binge eating and purging) are all part of the solution. Getting prescription strength fluoride
from your dentist to harden and desensitize your teeth and maintaining your oral hygiene by brushing and flossing at least twice a day is also crucial. if you regurgitate or vomit, rinse your mouth out right away. Adding baking soda to your brushing can also neutralize the acidic environment.
Regarding dental treatment, additive composite
bonding
to cover exposed dentin
or to replace lost tooth surface is a possibility. Getting a custom-fitted night guard to prevent further loss of tooth structure from bruxism and maybe veneers
or crowns to restore your mouth to ideal health are other possibilities. Of course, a professional assessment would be the first step of the process. The earlier you start treating the problem the better. A healthy mouth is essential for a healthy life.

or potassium), and brushing and flossing twice a day. Home fluoride rinse or gel can be used to desensitize and remineralize your teeth. A night guard
can be custom fitted to your mouth to protect your oral structure (worn while you sleep). A proper diet (eliminating sweets and/or acidic foods and beverages) will help prevent sensitivity. Your dental professional can work with you to harmonize and balance your bite so all teeth receive proper loading force as well – a proper fit and selection of dental restorations will assure integrity of pulpal health.And there is possibility for root canal therapy or Gum treatments if the problems are more advanced.
